All of our programs require Team Survivor Northwest membership.

Just follow the free two step process below, and we’ll get you moving!

Fill out a Membership Form

New Members must fill this form out completely and submit to TSNW. Returning Members must indicate on the form if they intend to remain active members each year and submit the form. Returning members do not need to update information on the form unless it has been changed. If you have any questions regarding our membership policies, please reach out to the TSNW Office at 206-732-8350.

While every effort is made to keep your information safe, privacy is not guaranteed. If you are more comfortable printing the form and sending it to us by mail or fax, just download the form and return it to TSNW. You may scan and e-mail it to us, fax it or send via snail mail at the addresses below.

Complete a Medical Release

PLEASE NOTE: All new and returning members must submit a new medical release in order to participate in TSNW activities in 2018.  After this year, returning members will only need to submit a new release if your medical condition has changed. In order to receive a lifetime membership card and to participate in TSNW activities, all new members must submit a medical release, signed by a physician.  

All medical releases must be signed by your physician. You may scan and e-mail it to us, fax it or send via snail mail at the addresses below.
