Fill out a Membership Form

New Members must fill this form out completely and submit to TSNW. Returning Members have a shorter form to be sure your contact information is current each year. Code of Conduct

Our priority is to ensure that Team Survivor Northwest is a safe and welcoming environment for all. By becoming a member, you agree to the TSNW Code of Conduct. Click here to read the Code of Conduct.

Upload Proof of COVID vaccination

All new and returning members who would like to participate in in-person programs and events must provide proof of COVID vaccination. You must be 2 weeks out from your primary series COVID vaccination of two doses (or 1 dose for J&J). Booster shots are no longer required.

Note: Scent-free Environment

Due to the sensitivities of those who have gone through chemo treatment, Team Survivor has a policy that prohibits scented products in order to provide a comfortable, scent-free environment for all members. Thank you for avoiding wearing products like perfume and scented lotion when you attend TSNW programs and events.
