All of our programs require Team Survivor Northwest membership. Membership is FREE!

Just follow the free two step process below, and we’ll get you moving!

Fill out a Membership Form

New Members must fill this form out completely and submit to TSNW. Returning Members have a shorter form to be sure your contact information is current each year. Please select the option below that applies to you. If you have any questions regarding our membership policies, please reach out to the TSNW Office at 206-732-8350.

Complete a Medical Release

All new and returning members must have a medical release on file in order to participate in TSNW activities in 2020.  Since 2018, returning members will only need to submit a new release if your medical condition has changed. In order to receive an annual membership card and to participate in TSNW activities, all new members must submit a medical release, signed by your medical care provider.  

Note: You do not need to visit your physician in order to have them sign the form. If you have seen them in the last year, they can base it off of your medical records and fax us the release directly. The brief form can be scanned & e-mailed, faxed or sent via mail at the address below.
