Scholarship Form
Guidelines
Team Survivor Northwest is committed to a thriving, diverse community. Scholarships help provide and increase access to outdoor programs and fitness activities for Team Survivor members.
Scholarships are awarded to members to participate in Team Survivor outdoor programs and fitness activities or to access gear to participate in these activities (e.g. hiking poles, shoes, backpack).
General:
- Up to 90% financial aid provided by Team Survivor per activity. Separate applications are required for each activity registration, and scholarships are based on availability of funds.
- Up to $100 per gear request (maximum $300 per season) based on availability of funds. Member must submit receipt(s).
- Recipients must be current members of Team Survivor and demonstrate financial need.
- Team Survivor may also support access to gently-used gear via our lending library.
1. Priorities: Awards will prioritize women who are currently in treatment or recently finished and/or who demonstrate financial need.
2. Non-transferable: If you cancel your registration for an activity for which you received a scholarship, those funds will be returned to Team Survivor. Participants must apply separately for assistance for other activities.
In the case of gear reimbursement, we strongly encourage you to attend several (3+) Team Survivor outdoor activities during the season. We do understand that health fluctuates; still, not attending activities after award may impact consideration for future financial aid requests.
3. Thank You note: If you receive financial aid, please consider sending a “Thank You” note to Team Survivor. “Thank You” notes that include photos are highly encouraged. These notes help us show the impact of the program to our donors.
These guidelines help us distribute financial aid across programs and to various individuals.
