TSNW MEMBERSHIP

Getting started at Team Survivor Northwest is simple. 

  1. Fill out a membership form. Current and new members must submit a membership form each year that includes signing the TSNW liability waiver. You can do this here via our on-line secure form (see on-line form below). Or you can download the form, print it, complete and either send it to us via snail mail; scan and e-mail; or fax it to us. See below to download the form and details regarding where to send it.
  2. Complete an Annual Medical Release. In order to participate in TSNW activities, all members must submit a new medical release every year. You will not receive your annual membership card, which enables you to participate in TSNW activities, until you submit your medical release. All medical releases must be signed by your physician. You may scan and e-mail it to us, fax it or send via snail mail.  Click on this link to get a copy of the form: 2017-medical-release-form.

ON-LINE MEMBERSHIP FORM: Please use this form to submit your information, on-line. Once it has been filled out you will receive an e-mail asking you to confirm your membership. Please remember to also print the medical release above and complete with your doctor.
